Replacement Tax form request during tax season January 1st – June 1st will be processed within 14 business days, any request submitted outside of this time period may take longer.
Any request submitted for the current year prior to November will be voided.
W2G Request– Only select the locations that you actually won jackpots at. If we process (2) locations without finding jackpot wins your request form will be voided and not processed any further. The original form was given to you upon win and reprints will be in a summary sheet only
If you need tax withheld amounts you must request that in the comments.
Win/Loss Request can be obtained instantly at the kiosk on any of our casino floors with your players club card. If you chose to request them through our office it falls within the same processing time as other tax forms.
YOU MUST LIST YOUR PLAYERS CARD NUMBER IF YOU ARE REQUESTING A WIN/LOSS FROM OUR OFFICE OR YOUR REQUEST WILL BE VOIDED
YOU MUST ONLY SELECT THE LOCATIONS YOU PLAYED AT- IF WE PROCESS (2) LOCATIONS WITHOUT PLAY YOUR REQUEST FORM WILL BE VOIDED
Lucky Star is not under a deadline to replace these forms as they have already been given to you upon win.
We DO NOT process walk-in request.
All replacement copies will be sent through the US Postal Service.
