Replacement tax form requests during tax season January 1st – June 1st will be processed within 14 business days. Any request submitted outside of this time period may take longer.

Lucky Star Casino offers replacement copies as a courtesy and there are strict requirements to ensure they are processed correctly.

 All customers needing a replacement tax form must complete the form on our website. No exceptions. Any customer needing a win/loss statement can obtain one instantly at out self-serve kiosks on the casino floor.  Win/loss statements requested here will take up to 14 days to process.

Due to the high rate of identity theft we will not send these forms without proper verification.

We do not take requests over the phone or in person.

 W2G Replacement Request– Only select the locations in which you won a jackpot. If we process (2) locations without finding jackpot wins your request form will be voided. The original form was given to you upon win and reprints will be in summary form by location. If you need tax withheld amounts please request so in the comments.

Win/Loss Statement Request: Can be obtained instantly at any kiosk on the casino floor with your players club card or be requested digitally here.  To request here please submit your players club card number in the comments. If you need your players card number please visit our Player’s Club office at any Lucky Star Casino location for assistance. Please only select the locations in which you played. If we process (2) locations without play your request will be voided.

 All requests are processed in the order they are received and take up to 14 days to process. 

 Once your request has been submitted and accepted you will receive an automatic confirmation email confirming our receipt. If you do not receive an email within an hour your request was not accepted and you need to re-submit the form. Please double check all information before re-submitting. If you receive your confirmation email but do not receive your tax forms within 14 days please “reply all” to your confirmation email and let us know it has not been received.  This is the quickest way to follow up on your request. You can also call Amanda Pawnee at 405-276-3173 after the 14 day period has passed.

 Due to the personal information on these forms, replacement copies are only processed by Audit Manager Amanda Pawnee or Audit Supervisor Amber Randall.

Lucky Star is not under a deadline to replace these forms as they have already been given to you upon win.

All replacement copies and statements will be sent through the US Postal Service.
